Before this partnership, Arvon CIN identified critical gaps in transitions of care and the substantial costs associated with preventable hospital readmissions. With nearly one in three patients being readmitted within 30 days of discharge, the need for more proactive management and monitoring was urgent.
Leveraging Puzzle Healthcare’s care management and post-discharge monitoring expertise, Arvon CIN implemented enhanced care coordination, physiatry integration, and advanced patient monitoring technologies. These efforts bridged gaps in care continuity, significantly reducing preventable readmissions and controlling excessive costs. Second, after discharge, Puzzle Healthcare’s multidisciplinary care management team monitors patients for 90 days using advanced tracking tools and disease-specific assessments, preventing complications that might lead to readmissions.
