(1) The transactions reported on this Form 5 were from an exercise of stock options that occurred in 2004 and previously reported on reporting person's Form 4; however the sale of the stock option exercise from the common stock was inadvertently not reported. Thus, reporting person is reporting the sale of the common stock that occurred in 2004 to rectify the error that occurred in 2004.
